THE ROLE –
Conduct Comprehensive Needs Assessments: Assess individuals and families’ housing situations, mental and physical health needs, and other relevant factors to develop personalized support plans.
Provide Case Management Services: Offer ongoing support, guidance, and advocacy to help clients achieve their housing goals and address other needs.
Facilitate Crisis Response: Respond to immediate housing crises, providing timely assistance and support.
Develop Capacity Building Plans: Assist clients in developing skills and resources to prevent future homelessness.
Collaborate with Community Partners: Work with other agencies and organizations to provide comprehensive services and support to clients.
THE IDEAL CANDIDATE – what you will bring.
Demonstrated ability and experience in leadership including the capacity to lead and support the team and individual staff.
Demonstrated experience and knowledge of case management practices.
Previous experience in crisis intervention and assessment with strong conflict resolution skills.
Valid Victorian Driver’s Licence |Working with Children Check |Police Check
Minimum bachelor’s degree in social work (or trauma related field) and experience appropriate to the field (refer to Position Description).
