Overview:

The Youth Focused Housing Pathways program provides brief intervention case work to young people

YFHP program is supported by Creating Connections, a program which aims to provide case management support to young people experiencing homelessness or who are at risk of homelessness, to secure independent living options , enhance living skills capacity and connection to community

About the program:

The YFHP program provides six weeks of intervention case work to young people who are:

  • First time homeless and not already receiving services from the homelessness service system
  • Young people who may be at risk of homelessness, and able to benefit from a youth-specific early intervention to halt any progression into the homelessness service system

This program is voluntary and requires ongoing, active participation from the young person

Eligibility:

A young person aged 16 to 25 years who are experiencing homelessness or, are at risk of homelessness residing in Latrobe City
