Homelessness Access Program
About the program

The Homelessness Access Program objective is to improve the access to primary healthcare services for people experiencing or at risk of homelessness, with a focus on people sleeping rough, in Latrobe, and offer assistance and practicable support with accessing primary health care services to Gippsland PHN Partnership providers.

The role will also work to improve the patient experience of care by increasing access to primary healthcare services and meeting patient needs, improve the provider experience via provider reported experience measures, improve healthcare cost efficiency by reducing avoidable hospitalisations, improve the health equity by providing fully subsidised primary healthcare services and improve health outcomes by reducing the disease burden

Objectives to the program are to:
  • Improve the patient experience of care by increasing access to primary healthcare services and meeting patient needs.

  • Improve healthcare cost efficiency by reducing avoidable hospitalisations
  • Improve the health equity by providing fully subsidised primary healthcare services

Eligibility criteria:

People experiencing recurring homelessness and sleeping rough in Latrobe Local Government Area (LGA) and need support to access healthcare to reduce hospital stays
