Creating Connections (CC)
Overview:

The Creating Connections Program focuses on providing clients with longer term support to assist clients to achieve more sustainable outcomes.

This work also includes the coordination and delivery of living skills programs designed to maximise the success of young people in taking up sustainable independent living. The team is currently providing a life and living skills program. Please ask your worker for details.

The Creating Connections Program is a consistent and coordinated approach to supporting young people aged 16-25, who are homeless or at risk of becoming homeless.  Creating Connections aims to provide a flexible and holistic approach
