To directly provide young people who are homeless, or at risk of becoming homeless, with intensive support. This support is delivered within a case management framework involving an assertive outreach approach. You will be responsible for a caseload of clients and will provide intensive transitional support to young people referred to the Youth Homelessness Service. You will assist clients to identify goals and work with them to achieve independent living. To provide a service which facilitates assertive outreach, assist client engagement and provide clients with information and referral to a broad range of community resources.
You will be a part of the Young People’s care, supervising and guiding them through activities in daily living and addressing their homelessness. You will be supporting their individual needs and creating a safe living environment. You will also be part of developing, implementing and monitoring their individual plans. This includes the maintenance of progress notes and reports. You will also provide specific interventions and services in relation to living skills, education and training, and access to the private market (including intensive case management).
