The Targeted Care Packages Program aims to achieve better outcomes for children and young people in out-of-home care. Decisions for Children and Young People will be focused on the child’s best interests, and will involve the application of the Best Interests Principles of the Children, Youth and Families Act 2005. In this role you will provide complex case management to children, young people and their families to support their exit from Out of Home Care into a variety of accommodation settings and deliver on identified goals as set out within each Targeted Care Package.
You will draw on your experience in case management of complex children, young people and families including leading and facilitating active multi-disciplinary care teams and the support and supervision of carers. Have a demonstrated knowledge and understanding of placement and support services, and Family Support Services. Diploma or above in Community Services or equivalent tertiary education.
