Youth Homeless Outreach Worker – 2 positions
SCHADS Award Level 5.
- 1 x Full time ongoing;
- 1 x 0.6EFT (22.8hours week) for a fixed term for 12 months.
About the role
This role is exciting and challenging, but the rewards are great! You will have the opportunity to support Young People who are homeless or at-risk of homelessness with intensive support. This support is to be delivered within a case management framework involving an assertive outreach approach.
You will be a part of the Young People’s care, supervising and guiding them through activities in daily living and addressing their homelessness. You will be supporting their individual needs and providing skills in creating a safe living environment. You will also be part of developing, implementing and monitoring their individual plans. This includes the maintenance of case notes and reports. You will also provide specific interventions and services in relation to living skills, education and training, and access to the private rental market (including intensive case management).
